DataBooks and CHSR Overview & Details*
DataBooks – Are excel spreadsheet files that include rates and trends for key MCAH indicators. Data are given at the county and region level and include comparisons to statewide rates.
DataBooks have multiple pages of data and information, including County/Regional numbers, State numbers, definitions, data quality, rates, and graphs, for different racial/ethnic groups and across several years. We strongly encourage you to consult the materials from the County Data Spreadsheets webinar training before using the DataBooks as well as the publications: DBK2018 - FINAL Title V 5-Year Needs Assessment Indicators: DATABOOK 2004 to 2015 and Trend Analysis Examples - Updated 2022 (a shorter overview). Please read these documents that pertain to important changes in methods for calculating some indicators derived from the birth certificate. Contact FHOP with any questions.
Community Health Status Report (CHSR) Overview – Is a spreadsheet with all of the data indicators you see listed on your county DataBooks page, and includes overall LHJ rates, state rates, confidence intervals when available, comparisons to the state rate when available, and the time period covered. The CHSR Overview is produced every 5 years for the Title V needs assessment. For reference, please see this document on CHRS Indicators and Data Sources.
Community Health Status Report (CHSR) Details – Is a spreadsheet that includes a subset of the data indicators that you see listed on your county DataBooks page that FHOP analyzes and produces the DataBooks for. The CHRS Details includes more in-depth comparisons of your current LHJ rates with your past rates, with the state’s current rates, with Healthy People (HP) 2020 goals when available, and a summary of trends when available that include the period in years, the rate, and confidence intervals. In the CHSR Overview you'll see a basic comparison between your county and the State and in the CHSR Details you will see a trend compared to the State's trends. Every time you see: CHSR only, please link back to the CHSR Overview or CHSR Details.

DataBooks labeled "PROVISIONAL": Because of the conversion from ICD-9-CM (International Classification of Diseases, Ninth Revision, Clinical Modification) to ICD-10-CM, for certain hospital discharge data there will only ever be 9 months of data for the year 2015. So all DataBooks labeled "Provisional" only have data through September of 2015. This will affect the number of admits you see for 2015, as the number is only for 9 months instead of a full year.

FHOP has produced 3 race/ethnic data quality DataBooks for local health jurisdictions for birth certificate data, hospital patient discharge data and hospital emergency department data. These DataBooks show rates and trends for UNDEFINED race/ethnicity for data from each of these sources. LHJs can use this data to monitor if their rates of UNDEFINED race/ethnicity exceed the Centers for Disease Control standards and to what degree.
Population Data
These files contains population data from the California Department of Finance (DOF). Data were summarized into age groups two ways, as DOF reported, and bridged into 5 longitudinally consistent race/ethnic groups.

These zip files contain shape files that you can use with epidemiological mapping programs.
